In-Home Nursing and Physical Therapy

Star Light Home Health is to enable patients under our care to live independently and retain a sense of freedom of choice as they age. Our specialist works closely with the patient, their physician and family to improve care coordination and communication using a team-based approach. As a fully insured and licensed medical home care agency, we offer a wide range of home health care services to families in Montrose, Colorado and surrounding counties. Our in-home care service can provide everything from skilled nursing to physical therapy and rehab therapy.

At Star Light Home Health, we take a proactive approach to care, closely monitoring each client to ensure that we catch care for any emerging issues before they become emergencies.

All of our care services are supervised with frequent care assessments by a licensed nurse in order to best provide for changing needs.


Registered Nurses

A home health care nurse provides one-on-one care for patients in their homes. These patients can be elderly, critically ill, or disabled, or they may be recovering from a surgery, injury, or accident. Our nurses are case managers that coordinate the care and supervise other disciplines according to doctor’s orders.


  • Assesses patient needs and develops plan of care with physician.
  • Coordinates and oversees care plan with patient, family, and support staff—LVNs and nurse assistants.
  • Monitors and evaluates patient response to treatment, medication, and healing or mobilization progress.
  • Oversees case management.
  • Assists with activities of daily living (ADLs)—mobility, bathing, grooming, toileting, etc.
  • Administers medication and intravenous infusions.
  • Takes vital signs, draws labs, and manages wound care.
  • Tends to wounds and other needs of home health patients.
  • Works with clinical healthcare professionals to coordinate home care.
  • Performs physical assessments.

Physical Therapy

Physical Therapist (PT) professionals evaluate and treat abnormal physical function related to an injury, disability, or other health condition.


Physical Therapist diagnose and treat a range of injuries, disabilities, and health conditions to improve a person’s range of movement and quality of life and prevent further injury or disability.

Occupational Therapy

The goal of occupational therapy is to improve the skills that are needed to live life as independently as possible.


Occupational therapy can help you to cope with a (new) mental or physical limitation and manage better in everyday life. This can improve your quality of life and health.

Speech Therapy

Speech therapy begins with assessment to determine your needs and the best treatment. Speech therapy exercises can help you with speech, language, and cognitive communication.


Speech Therapy may also include retraining of swallowing function if an injury or medical condition, such as Parkinson’s disease or oral cancer has caused swallowing difficulties.
